Caterpie

First, you see a red antenna pop out right above your nose. Next your skin turns green, while your eyes grow much larger. Your arms and legs start to shrink to mere stubs, causing you to fall forward. You start to shrink to the size and shape of a normal caterpie. Once you finish changing, you are full caterpie in body and mind.
